Information Before Entering

DISCLOSURE FORM FOR PRE-DISCHARGE BANKRUPTCY CLIENTS
CCCS OF SOUTHWEST GEORGIA has over 19 years of experience educating consumers and clients about money and credit. This agency is a member of the NATIONAL FOUNDATION FOR CREDIT COUNSELING. The NFCC has high standards for quality credit counseling and financial education, and this agency complies with those standards. in addition, this agency is accredited by COUNCIL ON ACCREDITATION (COA) an independent third-party organization that reviews and monitors entities that provide social services. We are a non-profit agency. We are organized and operate in accordance with Section 501 (©) (3) of the Internal Revenue Code. We do not pay or receive fees or other considerations for referrals of debtor students to our agency.

You have the option of taking Money in Motion, our bankruptcy and community education program, in a classroom setting, or online. A certified consumer credit counselor or teacher will be conducting or supervising the Money in Motion classes. Our instructors have completed the certification program offered by the NFCC or have a state issued teaching certificate. For online students, please email any questions you have regarding Money in Motion to predischarge@bellsouth.net. You will receive a response to your questions within 24 business hours.

We may ask you to voluntarily fill out basic demographic information. We will not disclose any such information that you provide orally or in writing to anyone, except as authorized by you in writing or as required by law. We may compile data in aggregate form; however, your information will not be disclosed in any manner that personally identify you.

To help cover the costs of providing this course to you, this agency charges a fee of $20.00 per individual for the on-line course and $50.00 per individual for the in-person course. In limited circumstances you may be eligible to have this fee waived. We will not deny service due to an inablity to pay.

At the conclusion of Money in Motion, you will be provided with a certificate that you will need in order to be discharged from your bankruptcy. You will need to provide us with your bankruptcy case number for this certificate.

If you are dissatisfied with the service provided by our agency you can utilize the Complaint Resolution Process. You agree to hold CCCS OF SOUTHWEST GA, its employees, and volunteers harmless from any claim, suit, and action resulting from this service. Nothing herein shall apply to actions or claims under the provisions of the United State Bankruptcy code, 11 U.S.C. Sec. 101 et seq.

To be actively involved in a comprehensive assessment of their financial situation including an appropriate action plan
To express dissatisfaction through a Complaint Resolution Process
To discontinue their relationship with CCCS at any time
To ask questions and to have concerns addressed

Complaint Resolution Process
We are commited to providing you high quality professional services. However, if you are not satisfied with the services provided or if you want to make a complaint, we ask that you follow these guidelines.

• Step One: Try to resolve the issue with the staff member involved - giving him or her specific information about your complaint.
• Step Two: If Step One is not possible or the issue is not resolved to your satisfaction, request a complaint/grievance form from any staff member at CCCS, 409 N. Jackson St., Albany, GA 31701. Complete and return the complaint grievance form to CCCS.
• Step Three: CCCS may request a meeting with you (phone or face-to-face) or seek more information from a staff person. CCCS will respond within 15 days.
Step Four: If your issue is still unresolved, you may appeal in writing directly to Irma Whitten, the Chief Executive Officer of CCCS. After additional fact finding, the C.E.O. will provide a concluding decision to you within 15 days.
